Hypertension

Although Blood pressure can be individual-specific, a reading of 140/90mmHg or higher is considered hypertension (150/90 mmHg for those aged 80 years or more). It can put individuals at high risk of cardiovascular diseases. 

COPD

Chronic obstructive pulmonary disease (COPD) is a life-threatening condition that affects airflow from the lungs. It is presented with breathing issues, coughing, wheezing and extreme fatigue. Smoking and pollution are the main causes of COPD. 

Heart failure

In the UK, about a million people, mainly adults, suffer from heart failure. About one-fifth of heart failure patients may need readmission within a month after discharge from a hospital.

Rheumatoid arthritis

Diabetes mellitus

Diabetes mellitus is a life-threatening disease in which the blood glucose level can abnormally rise. Here, the blood glucose level needs to be regulated to avoid complications of diabetes mellitus.

Frailty

Frailty includes a decline in both the physical and psychological ability of an individual for fast recovery from different health issues. One in every 10 individuals aged 65 years or above is subject to frailty syndrome, as per Age UK.
